This online form should only be used to notify us of the death in service of a member. If the member was retiring due to ill health, please contact us first to confirm the action that should be taken.

You will receive an email receipt to your account email address containing the details you submit on this form. Apart from the member's payroll number and job ID all personal details will be redacted. The details for the personal representative will also be redacted. Please ensure no identifying information is added to any fields for additional comments. If you have any resulting queries, we will return your form by secure email which will require a Barracuda account.
